Improve studydocument upload form

Some improvements to the studydocuments upload form:

• Adds suggested values (fetched from the API) to some fields of the studydocuments upload form and prevents accidental typos (one has to declare explicitly to create a new distinct value).
• Improves error feedback
• Improves page layout
