Add uploaded/updated at label to documents

It would be helpful to have an indicator of when a document was uploaded or last updated, so that you can figure out if there have been some changes.

Currently documents don't have a time/lastEdited field. This would have to be added to the model via a migration similar to how it's handled in answers.

Something in this direction, but a bit more in the matching style with how it's shown in answers.
