Duplicated trapz gradients in pulseq sequence files

The pulseq output files comprise duplicates of gradient waveforms (e.g. readout gradient that does not change from TR to TR).

This renders the resulting pulseq file unnecessary large and may cause problems when executing it on the MRI.
